In the first 16 months after a positive HPV result, only 43.7% underwent initial surveillance testing. Among these, 18.2% showed HPV-negative results, while 25.5% had abnormal results. Many patients, accounting for 49.4%, remained untested during this period, highlighting gaps in follow-up care.

The issue of follow-up care for high-risk HPV patients is critical, particularly with guidelines set forth by the American Society for Colposcopy and Cervical Pathology (ASCCP). The guidelines recommend annual surveillance for those with positive HPV and negative cytologic findings, emphasizing the need for timely testing to prevent cervical cancer. This study underscores the challenge in adhering to these recommendations across various demographics, revealing specific groups underrepresented in retesting.

The study concluded that adherence to guidelines for retesting patients with high-risk HPV is alarmingly low, particularly among marginalized groups. There is an urgent need for healthcare systems to improve surveillance processes and address social determinants of health that contribute to disparities in follow-up care.
